Job Summary

We are seeking a highly skilled and compassionate Care Management Supervisor to join our team at Healthcare in Action. As a key member of our care management team, you will be responsible for supervising and leading non-clinical staff in coordinating patient care, developing individualized care plans, and delivering ongoing care coordination and care management services.

Key Responsibilities
  • Supervise and Lead Care Management Team: Provide guidance, support, and oversight to non-clinical care management staff to ensure high-quality patient care and services.
  • Develop Individualized Care Plans: Utilize evidence-based guidelines and clinical knowledge to develop comprehensive care plans that address the unique needs and goals of each patient.
  • Deliver Ongoing Care Coordination and Care Management: Coach, educate, navigate, and refer patients to appropriate care and community-based resources to support their health and well-being.
  • Build Cross-Functional Relationships: Develop and maintain strong relationships with housing and allied community agencies to ensure seamless coordination of care and services.
  • Document Patient Information: Complete timely and accurate documentation in multiple computer systems to record assessment and corresponding documentation, including care plans and progress notes.
Requirements
  • Education: Bachelor's or Master's Degree in Social Work, Gerontology, or related field of study.
  • Experience: 1 year of people leadership with some direct supervision experience and 2-3 years of experience in homeless care management field with a Bachelor's Degree, or 2+ years of experience with a Master's Degree.
  • Skills: Strong interpersonal, organizational, and analytical skills, with excellent written and verbal communication skills.
