Hospice Care Specialist

Agape Care Group is seeking a compassionate and dedicated Registered Nurse to provide high-quality care to patients and their families facing life-limiting illnesses.
Key Responsibilities:
Create personalized care plans and evaluate patients to ensure a supportive and comfortable experience.
Communicate effectively with the care team and family members to address their needs and concerns.
Participate in ongoing education and training to stay up-to-date on best practices in hospice and palliative care.
Qualifications:
Graduate of an accredited school of nursing with a current state license as a registered nurse.
2+ years of nursing experience in a clinical care setting, with hospice experience preferred.
Reliable transportation and ability to meet physical requirements.
