Are you a dedicated and compassionate Registered Nurse seeking a rewarding career in end-of-life care? We are seeking a skilled Hospice Registered Nurse Case Manager to join our team at Interim HealthCare of Louisiana.

Key Responsibilities:
  • Provide home-based care to patients with six months or less to live, focusing on quality of life
  • Work collaboratively with an interdisciplinary team to carry out the patient's plan of care
  • Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
  • Educate family members on the patient's prognosis, changes in care plan, medication, and hospice goals
  • Take a holistic approach to care that engages the patient's mind, body, spirit, and family
Requirements:
  • Graduate of an accredited nursing program and active RN license in Louisiana
  • Minimum of 1 year of hospice nursing experience
  • CPR certification
  • Valid Louisiana driver's license, auto insurance, and reliable transportation
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty, strong communication skills, and compassion
