Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Health Registered Nurse - Case Manager. As a key member of our care team, you will be responsible for managing and delivering patient care plans, collaborating with healthcare professionals, and providing exceptional patient care in the comfort of their own homes.

Responsibilities
  • Manage and deliver patient care plans, ensuring high-quality care and positive patient outcomes
  • Collaborate with healthcare professionals, including physicians, therapists, and social workers, to coordinate patient care
  • Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to meet their unique requirements
  • Provide education and support to patients and their families, empowering them to take an active role in their care
  • Document patient progress, maintaining accurate and detailed records of care provided
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in patient care
Requirements
  • RN license in the state of employment
  • Graduate from an approved school of nursing
  • One year of RN experience, with clinical competence in delivering quality patient care
  • Current driver's license, with ability to spend approximately 20% of work time driving to/from patient locations
  • Commitment to meeting critical deadlines for charting and documentation
  • Ability to self-manage time and schedule, prioritizing tasks and managing multiple responsibilities
  • Demonstrated experience with tablets, mobile phones, and EMR software
