Job Description

We are seeking an experienced Medical Care Coordinator (Pediatrics) to join our team. As a Medical Care Coordinator, you will be responsible for coordinating and providing high-quality medical care to patients in the area of clinical specialization.

Responsibilities

  • Conduct comprehensive physical assessments of patients.
  • Develop and implement individualized care plans for patients.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate sharing of information.
  • Manage and maintain accurate and up-to-date patient records.
  • Participate in quality improvement initiatives to enhance patient outcomes.

Requirements

  • A minimum of a Master's degree from an accredited School of Nursing or graduation from an accredited School of Physician Assistants.
  • An unrestricted license to practice as a Physician Assistant in the State of Texas or an unrestricted license to practice nursing in the State of Texas/current credentialing as an Advanced Nurse Practitioner by the Texas Board of Nurse Examiners.
  • Current certification in the area of specialty.
  • Current DEA for the purpose of writing prescriptions.
  • Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) through American Heart Association.
  • Current Basic Life Support Certification for Healthcare Providers through American Heart Association.

Salary Estimate

The salary for this position is approximately $120,000 per year, depending on experience.
