Job Summary

We are seeking a highly skilled and compassionate Social Services Director to join our team at Southland Health & Rehab. As a key member of our leadership team, you will be responsible for developing and implementing the Social Services Program, ensuring that our patients receive the highest level of care and support.

Key Responsibilities
  • Plan, develop, and direct the Social Services Program, ensuring that it aligns with the organization's mission and goals.
  • Coordinate and participate in family and patient activities, promoting social interaction, reality orientation, and intellectual stimulation.
  • Counsel patients and families, addressing emotional, mental, environmental, or physical limitations.
  • Complete the social service portion of the MDS within 14 days of admission, ensuring accurate and timely documentation.
  • Coordinate admissions and discharges, ensuring a smooth transition for patients and families.
  • Act as liaison with social, health, and community agencies, fostering partnerships and collaborations.
  • Facilitate Patient and Family Councils, promoting open communication and addressing concerns.
  • Assist with grievance resolution, providing support and guidance to patients and families.
  • Understand and apply appropriate documentation for Advance Directives, ensuring patient autonomy and dignity.
Requirements
  • Minimum of a Bachelor's Degree in Social Work or a related field, or equivalent experience in a supervisory capacity.
  • Proficient in using a computer, especially Microsoft Office, Excel, and PowerPoint.
  • Excellent organizational and prioritizing skills, with effective and professional verbal and written communication skills.
  • Ability to read and interpret documents, write routine reports, and speak effectively before groups.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
