Patient Services Coordinator

The Patient Services Coordinator plays a vital role in ensuring seamless care coordination and continuity for patients. This position requires strong communication skills, organizational abilities, and the capacity to manage multiple assignments.
- Key Responsibilities:
- Manage patient schedules, ensuring timely rescheduling and updates to agent unavailability.
- Initiate infection control forms as needed and send completed reports to HRD for upload.
- Provide backup support during peak hours, receiving calls from field staff and assisting with weekly case conferences.
- Maintain the client hospitalization log, including coordination notes and electronic logs for office, field, and sales staff.
- Complete requested schedules for skilled nurse/injection visits, ensuring staff are scheduled unless an aide supervisory visit is scheduled in conjunction.
- Process reassigned and rescheduled visits, ensuring supervisory visits are scheduled.
- Run scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report.
- Prepare weekly Agent Schedules, reviewing for productivity and geographic issues before forwarding to the Branch Director.
- Verify visit paper notes in the scheduling console as needed.
- Assist with internal patient transfers between branch offices.
- Perform lab report assessments, faxing copies to doctors and routing to Medical Records Department as necessary.
- Participate in on-call rotations and perform patient visits as required.
Requirements:
- Excellent communication skills and ability to interact with diverse individuals.
- At least 1 year of home health experience.
- Prior packet review/QI experience preferred.
- Coding certification is preferred.
