Job Summary:

Kaiser Permanente is seeking a skilled professional to join our team as a Mental Health Case Manager. As a key member of our care team, you will provide comprehensive psychosocial services to adult and child members in a primary or specialty care setting.

Key Responsibilities:

  • Evaluate psychosocial assets and deficits, and provide short-term counseling, education, and advocacy to enable and/or maintain the greatest level of independence.
  • Assess, triage, diagnose, treat, and/or provide crisis intervention services for members with mental health and addiction needs.
  • Collaborate with treating primary or specialty care providers, and with psychiatric, addiction medicine, and allied health professional team members to plan and direct each individual patient's treatment program.
  • Participate in departmental program development, implementation, and evaluation.
  • Develop, implement, coordinate, and evaluate plan of care and/or clinical treatment programs for members with acute or chronic mental or addiction illness.
  • Participate in staff conferences for care coordination, including selection, planning, and evaluation of plan of care and/or treatment programs.
  • Instruct and counsel patients and their families regarding compliance with plan of care.
  • Complete all documentation of the member's treatment and progress in accordance with Kaiser Permanente, state, and NCQA regulations, and in keeping with accepted community standards within scope of role and setting.

Requirements:

  • Masters degree in Social Work (or Social Welfare from a clinical track), Clinical or Counseling Psychology, Marriage & Family Therapy, or related field required from an accredited college or university.
  • License, Certification, Registration: Marriage and Family Therapist Associate License (Washington) OR Mental Health Counselor Associate License (Washington) OR Licensed Social Worker Associate-Independent Clinical (Washington).
  • Prior completion of a practicum or internship in a clinical setting.
  • Knowledge of the bio-psycho-social functions that contribute to mental health.
  • Commitment to quality of service, teamwork, and participation in a highly interactive multidisciplinary clinic.
  • Must be able to work in a Labor/Management Partnership environment.
  • Experience working in a medical setting with multi-disciplinary teams.
  • Experience working with electronic health record.
