Responsibilities:
  • Conduct initial and ongoing comprehensive assessments of patients and their families.
  • Develop and update patient care plans as necessary.
  • Utilize health assessment data to establish nursing diagnoses.
  • Administer prescribed medications and treatments as outlined in the care plan.
  • Provide counseling to patients and families to address nursing and related needs.
  • Maintain accurate clinical documentation.
  • Communicate effectively with physicians and participate in interdisciplinary meetings to address the medical and emotional needs of patients and their families.
  • Ensure adherence to all state and federal regulations.
  • Perform additional duties as assigned.

Qualifications:
  • Graduate of an accredited nursing program with current licensure in the state of practice.
  • Minimum of 1 year of nursing experience required; 3+ years preferred.
  • Experience in hospice or a similar environment is preferred.
  • Understanding of the hospice philosophy of care.
  • Commitment to excellence in clinical practice and documentation.
