Value-Based Care Coordinator LPN

We are seeking a skilled Transitional Care Coordinator LPN to join our team at Addus HomeCare. As a key member of our Value-Based Program, you will play a critical role in ensuring the highest level of care for our patients.
Key Responsibilities- Client Enrollment and Coordination: Receive incoming calls/referrals for clients enrolling in value-based programs and maintain accurate lists of enrolled clients.
- Initial Home Visits: Coordinate or perform an initial in-home visit with enrolled clients to assess their needs and develop personalized care plans.
- Medication Reconciliation: Ensure completion of a medication reconciliation for all enrolled clients to ensure accurate and safe medication management.
- Client Education and Training: Perform telephonic or in-person training to clients on their medical conditions, medications, and care plans to empower them to take an active role in their healthcare.
- Resource Referral and Development: Work with clients to identify gaps in in-home care and refer them for home health, hospice, or in-home physician services as needed. Develop relationships with extended services and refer clients as necessary.
- Change in Condition Management: Receive and document Change in Condition calls from the health engagement team and direct care staff, and perform follow-up as necessary.
- Process Improvement: Monitor internal processes for efficiency and validity, and make recommendations for improvement.
- Data Management: Manage data flow from operations and report outcomes to internal and external customers.
- Education and Experience: Current RN or LPN license with at least three (3) years of experience in a clinical setting, with previous experience in home care and home health preferred.
- Certifications and Skills: Possess and maintain valid CPR certification, and possess excellent verbal and written skills. Solid computer proficiency in Microsoft Office, including Excel and web-based applications.
- Personal Qualities: Committed to exceptional customer service and meeting customer needs, with the ability to complete tasks in a multi-phased, fast-paced environment.
