Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Dependable Health Services in the Tucson area. As an LPN, you will play a vital role in providing high-quality patient care and contributing to our organization's mission of delivering exceptional service and care to our patients and employees.

Key Responsibilities
  • Assess patient needs and provide supportive care in accordance with physician orders and the interdisciplinary group plan of care.
  • Collaborate with the Nurse Case Manager to coordinate optimal care and use of resources for patients.
  • Communicate effectively with attending physicians, staff members, and other agencies as needed.
  • Maintain regular communication with the Nurse Case Manager, Program Director, and Medical Director.
Requirements
  • Current licensure as an LPN/LVN in the State of Arizona.
  • Ability to work some after-hours and weekend shifts.
  • Excellent verbal and written communication skills, as well as organizational skills.
  • Valid driver's license and reliable vehicle.
  • Current BLS certification.
  • Ability to work in a fast-paced environment and adapt to changing situations.
Preferred Qualifications
  • Home Health experience.
  • Bilingual (English/Spanish) skills.
