Job Title: PT Care Partner ADL/QMAP for Senior Living Community

Job Summary:

We are seeking a part-time Care Partner ADL/QMAP to join our team at Kavod Senior Life. As a Care Partner, you will play a vital role in providing non-medical care and support to our residents, ensuring their comfort, dignity, and well-being. Responsibilities:
  • Monitor daily status of residents and execute individualized resident care plans
  • Provide non-scheduled cleaning of residents' apartments on a PRN basis during times housekeeper is not available
  • Assist with residents' linens, laundry, and ironing
  • Provide assistance with residents' bathing, grooming, and hair care
  • Monitor residents' medication routine as specified by State regulations and Kavod Assisted Living medication policies and procedures
  • Provide assistance for residents with dressing and undressing
  • Encourage and guide residents' development of self-care capabilities and personal hygiene
  • Assist with AL meals to assure a pleasurable dining experience for residents
  • Escorts residents to meals and building activities, as needed
  • Provide regular contact checks and reassurance for residents and document anything out of the ordinary
  • Maintain a positive attitude with residents, families, and community members
  • Perform additional agency custodial services as designated by the Director of Assisted Living
Requirements:
  • Must be 18 years of age or older with a high school diploma or general education degree (GED)
  • Must read, write, and speak English
  • Fulfillment of all training and other regulatory requirements (i.e. QMAP certification, First Aid/CPR training)
  • Experience providing personal care and housekeeping for elderly and/or certified nurse's aide training
