We are currently looking for a Practical Nursing Specialist to join our team. This role is essential in delivering comprehensive care to individuals in community environments, employing the nursing process of assessment, planning, intervention, implementation, and evaluation.

This position involves collaboration with individuals, their families, and healthcare teams while upholding the standards of professional nursing practice.

Compensation: Starting pay is $30.00/hour

Key Responsibilities:
  • Administer treatments and healthcare procedures as directed by a physician and in accordance with professional standards.
  • Ensure individuals attend their medical appointments as outlined in their Individual Support Plans (ISP).
  • Communicate any concerns or issues regarding nursing services to the supervisor.
  • Prepare and distribute medical reports that detail general health, healthcare objectives, and physician recommendations.
  • Collaborate with an interdisciplinary team to develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP), incorporating observations and behavioral developments.
  • Provide training to staff on the medical needs of assigned individuals.
  • Monitor the health and medical conditions of individuals, referring them for medical services as necessary.
  • Oversee medication administration through accurate logs.
  • Ensure the availability and replenishment of health service supplies.
  • Complete program documentation and billing records accurately and promptly.
  • Foster a support network for individuals by building and maintaining relationships with community members, businesses, and families.
  • Engage individuals in community, social, and recreational activities.
  • Participate in circles of support for personal futures planning.
  • Adhere to strict OSHA protocols regarding bloodborne pathogens.
  • Cooperate with licensing and departmental staff during inspections or inquiries.
  • Additional duties may be assigned as needed.
Minimum Qualifications:
  • Bachelor's Degree or equivalent experience in the nursing field.
  • At least one year of experience providing nursing services to individuals with disabilities.
  • Valid nursing license.
  • Valid driver's license with a satisfactory driving record.
  • Completion of all state and agency-required training per state guidelines.
  • Ability to calculate medication dosages accurately.
  • Commitment to community-based support for individuals with disabilities.
  • Ability to exercise sound judgment and remain composed in crisis situations.
  • Capability to establish supportive relationships with individuals receiving care.
  • Proficiency in Microsoft Office.
  • Excellent verbal and written communication skills.
  • Aptitude for effectively presenting facts and recommendations.
  • Ability to communicate effectively with program participants, third-party agencies, staff, and community members.
  • Strong time management skills.
Working Conditions:
  • Work in various settings including offices, residential programs, day programs, and community environments.
  • May encounter changing weather conditions when working outdoors.
  • Hours and schedules may vary based on business needs, including evenings and weekends.
  • On-call duties may be required in coordination with other management staff.
  • Occasional overtime may be necessary.
  • Frequent physical exertion may be required, including lifting and stretching.
  • May need to lift or move over 50 pounds.
  • Occasional assistance with wheelchair transfers for non-ambulatory individuals may be required.
  • Transportation of individuals may be necessary using personal or company vehicles.
  • Exposure to various medical conditions, communicable diseases, and bodily fluids may occur.
