Job Summary:

Assistance Plus is seeking a skilled Nurse Care Specialist to provide medical oversight to clients enrolled with our Behavioral Health Home Organization team.

The ideal candidate will have a strong background in clinical nursing and experience in rehabilitation services. This is a unique opportunity for a nurse looking for a standard schedule with no weekends, nights, or on-call duties.

Key Responsibilities:

  • Assessing the medical needs of clients and coordinating services based on these needs
  • Providing education on medical diagnoses and prescribed medications
  • Participating in assessments for higher levels of care, such as PSS, Skilled Nursing, and Assisted Living
  • Making referrals for high levels of care when applicable
  • Developing and implementing treatment plans with goals targeting medical needs
  • Providing backup coverage for the BHH Nurse Care Manager
  • Completing daily notes for direct and indirect client contact
  • Setting up Health Info Net patient lists and following up on notifications
  • Supporting and educating case management team members on client cases
  • Monitoring medication side effects and coordinating with providers to minimize negative effects

Requirements:

  • Proof of vehicle insurance and a valid Maine driver's license
  • Associate's Degree required, Bachelor's Degree preferred
  • Minimum three years of clinical nursing experience
  • Current license to practice in Maine as a Registered Nurse (RN), Advance Practice Nurse (APRN), Licensed Practical Nurse (LPN), or Clinical Nurse Specialist (CNS)
  • Intermediate-level computer experience
