Palliative Care Nurse Practitioner
Constellation Health Services is seeking a skilled and empathetic Palliative Care Nurse Practitioner to join our team. This role involves providing high-quality patient care in various settings, including patient homes and facilities.
Key Responsibilities:
- Conduct comprehensive assessments and coordinate care plans with interdisciplinary teams.
- Monitor patients' physical and emotional status, reporting changes to supervisors and physicians.
- Administer medications and treatments as prescribed, ensuring patient safety and well-being.
- Participate in weekly team meetings, providing updates on patient status and contributing to care planning.
- Maintain accurate documentation, adhering to agency standards and regulations.
- Utilize electronic documentation systems, submitting required information in a timely manner.
- Act as a liaison between institutions, promoting continuity of care.
- Represent the agency in community activities, fostering relationships and promoting our services.
Qualifications:
- Master's degree in nursing from a CCNE-accredited university.
- Current license to practice nursing in New Hampshire and/or Maine.
- National Board Certification as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP).
- Experience in hospice or palliative care, clinical oncology, geriatrics, or intensive care.
- Palliative care certification (NBCHPN) or hospice certification (ACHPN) preferred.
