MDS Nurse Coordinator

As a Minimum Data Set (MDS) Coordinator - RN, you will be instrumental in achieving the business goals of Country Village Center by ensuring accurate reimbursement for patient care services. Your responsibilities will include participating in the Patient Assessment Process, which involves completing items on the MDS and the associated Care Area Assessments (CAAs).
Key Responsibilities:
- Assist in formulating individualized patient care plans, identifying patient needs and expected outcomes in compliance with Federal and State regulations.
- Contribute to the interdisciplinary Resident Assessment Instrument (RAI) process, adhering to RAI guidelines for completion, computer encoding, and electronic submission of the MDS.
- Recognize changes in patient conditions alongside the interdisciplinary team and coordinate the scheduling and execution of necessary assessments.
- Provide training on Activities of Daily Living for new hires and facilitate ongoing education for existing staff.
- Participate in educational programs to stay updated with current RAI/Prospective Payment System regulations.

- Must possess a current Registered Nurse license in good standing.
- Prior experience in a hospital, long-term care facility, or other healthcare environments is essential.
- Experience in completing MDSs and Care Plans is required.
- Knowledge of general, rehabilitative, and restorative nursing practices, as well as laws and regulations governing long-term care, is necessary.
