As an MDS Nurse Coordinator, you will be instrumental in achieving our business objectives by ensuring accurate reimbursement for patient care services. Your responsibilities will include participating in the Patient Assessment Process through the completion of the Minimum Data Set (MDS) and the associated Care Area Assessments (CAAs).

Key Responsibilities:
  • Assist in developing individualized patient care plans while identifying patient needs and projected outcomes in compliance with Federal and State regulations.
  • Contribute to the interdisciplinary Resident Assessment Instrument (RAI) process, adhering to RAI guidelines for MDS completion, computer encoding, and electronic transmission.
  • Recognize changes in patient conditions alongside the interdisciplinary team and coordinate the scheduling and execution of necessary assessments.
  • Provide training on Activities of Daily Living for new hires and ongoing education for existing staff.
  • Participate in educational programs to stay updated with current RAI/Prospective Payment System regulations.

Qualifications:
  • Must be a Registered Nurse currently licensed by the state and in good standing.
  • Previous experience in a hospital, long-term care facility, or other healthcare-related environments is required.
  • Prior experience with MDS and Care Plans is essential.
  • Knowledge of general, rehabilitative, and restorative nursing practices, as well as laws and regulations governing long-term care, is necessary.
