Community-Based Wellness Coach

**Job Overview:** The Mental Health Center of Greater Manchester Inc. is seeking a qualified Behavioral Health Transition Specialist to join our care transition team. This role requires a highly skilled and compassionate professional who can provide community-based interventions to individuals with behavioral health challenges.
Responsibilities:
- Develop and implement personalized transition plans for assigned clients, using a person-centered approach.
- Collaborate with community agencies and natural support systems to ensure seamless coordination of services.
- Maintain accurate documentation of service provision.
- Participate in weekly organizational meetings.
Requirements:
- Bachelor's degree in a related human service field or equivalent experience.
- At least 2 years of experience working with behavioral health populations.
- Demonstrated knowledge of social and human service providers, public benefits systems, and referral processes.
Benefits:
- Competitive salary: $60,000 - $80,000 per year, depending on experience.
