Key Responsibilities:

  • Provide case management services to clients, including assessment, goal setting, and service planning.
  • Develop and maintain relationships with community resources and service providers to ensure access to necessary services.
  • Conduct home visits and provide in-home support services as needed.
  • Collaborate with other staff members to ensure comprehensive and coordinated services.
  • Maintain accurate and timely records of client interactions and services provided.

Requirements:

  • Minimum of one year relevant experience with homeless, mental health, and substance abuse services and resources.
  • Valid driver's license.
  • Proficient computer and technology skills, including Microsoft Word and Excel and other web-based applications.
  • Flexible, reliable, motivated, and possess a positive attitude.
  • Excellent time management skills and able to meet deadlines.
  • Organized, able to multi-task, and follow-through with tasks to completion.
  • Work well independently and as part of a team environment.

Compensation:

  • $23/hour
