Job Summary

The City of Somerville is seeking a Part-Time Case Manager to provide case management services to residents in need of support for their mental, social, and physical wellbeing. The successful candidate will work under the direct supervision of the Deputy Director of Health and Human Services and will be responsible for providing information to residents on resources and services available to them, making referrals to City colleagues and external partner agencies, and creating a safe and supportive environment for clients.

Key Responsibilities
  • Conduct assessments and intakes of clients in need of services and develop plans to address their concerns
  • Develop relationships and work with community-based organizations and other governmental agencies to identify resources and services for clients
  • Provide referrals to other community agencies regarding needs such as mental health, housing, homelessness, and substance use
  • Maintain client databases to track efforts and outcomes and produce required reports
  • Develop strong partnerships with community organizations and service providers
  • Conduct workshops for internal City staff and external partner agencies on subject matters related to case management and oversight
Requirements
  • Bachelor's Degree and three to five years' experience in case management services
  • Licensed Social Workers (LSW) are strongly encouraged to apply
  • CPR/First Aid Certification within three months of hire and continually maintained thereafter
  • Ability to travel independently to multiple locations within the City to complete work assignments
Working Conditions

The work environment involves everyday discomforts typical of offices, with occasional exposure to outside elements. Noise or physical surroundings may be distracting, but conditions are generally not unpleasant.
