Job SummaryThis is a casual position, working weekends (Saturday and Sunday) with flexible scheduling of patient visits. You will be responsible for providing high-quality patient care in the comfort of their own homes, working independently and as part of a collaborative team.
Responsibilities- Participate in ongoing interdisciplinary assessment of the patient
- Develop and evaluate the plan of care in partnership with the patient, representative (if any), and caregiver(s)
- Provide services ordered by the physician as indicated in the plan of care
- Recognize and report changes in the patient's condition to the RN supervisor
- Counsel and educate the patient and family in meeting healthcare-related needs
- Complete timely and accurate clinical notes
To be successful in this role, you will need:
- Current license as a Licensed Practical Nurse (LPN) in the State of MN
- 2 years' experience as an LPN
- Able to travel within the geographic area serviced by the office
- Valid driver's license, reliable transportation, and proof of auto insurance
- CPR Certification
