Key Responsibilities- Complete admissions and visits for patients in their home setting
- Provide skilled nursing care and support to patients and their families
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Document patient information and progress in a timely and accurate manner
- Graduate of an accredited school of nursing and licensed to practice as a Registered Nurse in the state of Minnesota
- Minimum of twelve (12) months experience working in an acute care or home health environment
- Evidence of current CPR certification
- Experience in Home Care, including OASIS, preferred
- Competitive annual salary range: $76,960 - $89,440
- $6,000 hiring bonus
- Mileage reimbursement and company car program

- 100% clinical license reimbursement
