Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Care team as an Admissions Nurse. In this role, you will be responsible for completing admissions and visits for our patients, providing high-quality care and support in their home environment.

Key Responsibilities
  • Conduct thorough assessments and develop individualized care plans for patients
  • Provide skilled nursing care and support to patients in their homes
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Graduate of an accredited school of nursing and licensed to practice as a Registered Nurse in the state of Minnesota
  • Minimum of 12 months experience working in an acute care or home health environment
  • Current CPR certification and reliable transportation
  • Ability to travel within a geographic area serviced by the office
