Job Summary: We are seeking a skilled Registered Nurse to perform admission and start of care visits on weekends in the Twin Cities area.
Responsibilities:
- Admission/OASIS Visits: Conduct admission and start of care visits in designated areas of the Metro.
- Client Evaluation: Assess clients for home health services and create their plan of care.
- Service Recommendation: Recommend services from other disciplines not yet ordered by the physician.
- Collaboration: Collaborate with clients, families, physicians, and other home care team members.
Requirements:
- Licensure: Current RN licensure in Minnesota.
- Driving: Reliable vehicle with current insurance and a valid Driver's License.
- Experience: Prior home health nursing experience.
- Documentation: Proficiency with Medicare & OASIS documentation.
- Autonomy: Demonstrated ability to work autonomously.
- Communication: Excellent planning, organizing, and communication skills.
