This role focuses on managing care for individuals with significant health needs, ensuring a comprehensive approach to clinical and medical coordination. The primary goal is to enhance patient outcomes by addressing barriers to treatment and providing necessary education regarding health conditions and preventive care.
Key Responsibilities:
1. Identify and assess high-need individuals for tailored care management services.
2. Facilitate care management services for eligible clients, maintaining a manageable caseload.
3. Collaborate with other care managers to address complex medical and behavioral health cases.
4. Work alongside senior clinical coordinators to identify and implement beneficial training for staff.
5. Monitor ongoing eligibility for care management services and maintain accurate documentation.
Essential Qualifications:
+ Strong organizational skills with a keen attention to detail.
+ Proficient problem-solving abilities.
+ Excellent communication skills, both verbal and written.
+ Clinical knowledge relevant to the populations served.
Education and Experience Requirements:
+ LMSW required; progress towards LCSW licensure preferred.
+ Minimum of three years of experience in care management for individuals with co-occurring disorders.
+ Basic computer proficiency and effective documentation skills.
+ Valid driver's license and access to a personal vehicle for community travel.
Physical Requirements:
+ Ability to work in an open office environment and remain mobile throughout the day.
+ Capable of using a computer for extended periods.
