POIC & RAHS is a leading provider of community-based services, dedicated to empowering individuals and families to achieve their full potential. We are seeking an experienced Care Manager to join our team.
**About the Role:
This dynamic position requires a highly skilled professional who can develop and implement strength-based individual success plans for adult males at risk of further penetration into the criminal justice system. As a Care Manager, you will work collaboratively with other professionals in the field to assess and address the unique needs of each client.
**Key Responsibilities:
* Assess and implement evidence-based case management plans that promote cultural sensitivity and support clients' goals*
* Facilitate workshops focused on life skills, education, employment, and relationships using the 'Black Male Success' curriculum*
* Collaborate with schools, community-based organizations, mental health services, and social services to provide comprehensive support*
* Develop and coordinate pro-social activities for adults and their families*
* Maintain accurate documentation and records according to POIC policies*
* Respond promptly to crises and risk incidents*
**Requirements:
* A bachelor's degree in a relevant field (social work, psychology, or a related field)
* At least three years of experience in case management, preferably working with at-risk populations*
* Excellent communication and interpersonal skills*
* Ability to perform duties with or without reasonable accommodation and without posing a direct threat to the safety or health of others*
