We are seeking a highly skilled Clinical Pharmacist to join our team at Advocate Aurora Health. As a Clinical Pharmacist, you will play a critical role in providing patient-centered care and ensuring the safe and effective use of medications.
Key Responsibilities- Develop and implement individualized medication therapy plans for patients
- Evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions
- Review and accurately interpret medication orders
- Develop competency in dispensing and compounding medications consistent with good pharmaceutical practice
- Practice in compliance with department policies & procedures, as well as local, state and federal regulations
- Ensure formulary adherence, recommend cost-effective therapies to providers, and minimize waste in daily practice
- Actively support procedures and initiatives to prevent medication events
- Report events through appropriate channels
- Actively support department medication safety and quality improvement activities
- Orient/train new pharmacy team members
- May work with interns, residents and students to guide daily activities
- May be preceptor-in-training
- Provides direction to pharmacy technicians to ensure smooth workflow and service standards
- Pharmacist license issued by the state in which the team member practices
- Bachelor's Degree in Pharmacy
- Hospital pharmacy experience preferred
This position requires the ability to stand for long periods of time, occasional lifting up to 35 lbs without assistance, and exposure to chemical hazards.
