About the Job

Pillar Care Continuum is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team in Wayne, NJ.

The ideal candidate will have experience in healthcare settings, strong knowledge of health regulations and first aid, and excellent communication skills. The LPN will be responsible for assessing and managing the health needs of consumers, administering medications, and providing essential healthcare support.

Job Summary

  • Assess daily health status of consumers, treat medical problems as appropriate, and notify the Service Coordinator of significant observations and concerns.
  • Administer prescribed medication to consumers and first aid to consumers and/or staff.
  • Maintain accurate and detailed medical records, including regular anecdotal notes and reports as necessary.

Responsibilities

  • Participate in initial evaluations by reviewing all pertinent medical records and assessing the physical well-being of consumers.
  • Develop an Individual Habilitation Plan by including health status and healthcare needs; participate in yearly re-evaluations.
  • Organize medical and dental screening clinics; participate in clinics as required by healthcare providers.

Requirements

  • LPN License required; graduate of an accredited nursing program licensed in the State of New Jersey.
  • Minimum 1 year experience in an Adult Program, Hospital, Nursing Home, Clinic, or Group Home setting or equivalent combination of education and experience.
  • Strong knowledge of health regulations, first aid, and emergency procedures.
  • Excellent communication and interpersonal skills, with ability to work effectively with consumers, guardians, and staff.
