We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team at Pillar Care Continuum in Wayne, NJ. The ideal candidate will have experience in healthcare settings, strong knowledge of health regulations and first aid, and excellent communication skills.

Key Responsibilities:
  • Conduct initial evaluations by reviewing medical records and assessing the physical well-being of consumers.
  • Develop and implement Individual Habilitation Plans that address health status and care needs.
  • Assess daily health status, treat medical problems, and notify the Service Coordinator of significant observations.
  • Administer prescribed medication and first aid as needed.
  • Maintain accurate medical records and write reports as necessary.
  • Organize medical screening clinics and participate as required.

Salary and Benefits:

$33.00 to $35.00 per hour
