Job Title: Referral Specialist

Job Summary:

The Referral Specialist will work with the Division of Family and Child Health to create and oversee programs, policies, services, and environments that support physical and socio-emotional health, and promote primary and reproductive health services, health equity, social justice, safety, and well-being for New York City families and children.

Responsibilities:

  • Collaborate with the Bureau of Maternal Infant and Reproductive Health, the Bureau of Early Intervention, the Office of School Health, and the Bureau of Administration to develop and implement programs and policies.
  • Provide referral services to families and children in need of health and social services.
  • Work with community-based organizations and healthcare providers to ensure access to health and social services.
  • Develop and maintain relationships with healthcare providers and community-based organizations to ensure effective referral services.
  • Monitor and evaluate the effectiveness of referral services and make recommendations for improvement.

Requirements:

  • A four-year high school diploma or its educational equivalent approved by a State's department of education or a recognized accrediting organization.
  • One year of satisfactory clerical experience.
  • Keyboard familiarity with the ability to type at a minimum of 100 key strokes (20 words) per minute.
