Job Title: Registered Nurse Case Manager

Job Summary:

The Registered Nurse Case Manager is a critical member of the interdisciplinary team at the Texas Health & Human Services Commission. This role requires a skilled and compassionate nurse to provide high-quality care to individuals with intellectual and developmental disabilities who are medically fragile or have behavioral problems.

Key Responsibilities:
  • Conduct comprehensive assessments of individuals' health status and develop individualized care plans.
  • Provide direct patient care, including administering medications, performing physical assessments, and documenting health status.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Participate in quality assurance reviews and quality improvement initiatives to enhance patient care and outcomes.
  • Develop and maintain professional relationships with individuals, families, and staff members.
Requirements:
  • Licensed to practice as a registered nurse in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact.
  • At least one (1) year experience as a practicing registered nurse is required.
  • Experience in assessment and documentation of health status is required.
  • Preferred qualifications include experience working with individuals with intellectual and developmental disabilities and a Bachelor's Degree.
