Job Summary

We are seeking a highly skilled Registered Nurse Case Manager to join our team at the Texas Health & Human Services Commission. As a Registered Nurse Case Manager, you will be responsible for providing complex nursing care to individuals residing at our State Supported Living Centers.

Key Responsibilities
  • Assess, care, and integrate treatment for individuals with intellectual and developmental disabilities
  • Oversee the delivery of nursing care and monitor health care status to meet the health care needs of individuals assigned to the caseload
  • Work closely with the interdisciplinary team to ensure consistent, high-quality care for all persons served
  • Perform physical assessments, document healthcare needs, develop health care plans, and oversee the implementing of nursing interventions
  • Review available documentation for issues and incidents involving assigned individuals and perform trend analysis of health care needs and/or issues
Requirements
  • Licensed to practice as a registered nurse in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact
  • At least one (1) year experience as a practicing registered nurse is required
  • Experience in assessment and documentation of health status is required
  • Preferred qualifications include at least two (2) years of experience as a practicing registered nurse in a health agency, hospital, or health-care facility, a Bachelor's Degree, and previous experience working with Individuals with Intellectual and/or Developmental Disabilities (IDD)
