Work Hours:
8am to 5pm

or

9am to 6pm

Work Setting:

Federally Qualified Health Center (FQHC)

Patient Population:
Young Adults & Adults

Patient Volume:
10 to 20 per day

Responsibilities:

The Substance Use Counselor will:
Assess patients' behavioral health needs
Provide substance use prevention education
Conduct screenings and brief interventions during and/or after medical appointments
Provide individual and group counseling
Refer patients with moderate to severe behavioral health needs to onsite Behavioral Health team or community mental health institutions
Refer patients with moderate to severe substance use to the onsite Medication Assisted Treatment program or to community partners specializing in detox, intensive outpatient and residential treatment
Perform other related work, as assigned

Qualifications:
2+ years of experience working with Adults with Dually Diagnosis and/or Substance Use issues
Current Licensed Chemical Dependency Counselor in good standing
Demonstrated significant knowledge of Behavioral and Primary Integrated Care models
Basic knowledge of Cognitive Behavioral Therapy based counseling methods and techniques
Basic knowledge of the Counseling 12 Core Functions of drug and alcohol substance use/abuse
Basic knowledge of 12-Steps programs and other support groups
Basic knowledge of case management
Solid assessment, clinical, and documentation skills
Patient oriented
Great interpersonal skills
Excellent communication skills (written and verbal)
Strong attention to detail
Highly organized

Desired Skills:
Graduate from an accredited college or university with a Bachelor's and/or Master's Degree in Counseling, Social Work, Behavioral Science or a closely related field
Experience working in a Medical setting
Experience working with Electronic Health Records
