About the Role:
We are seeking a skilled and compassionate RN MDS Coordinator to join our team at Touchstone Communities. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality of care for our patients and residents.
Responsibilities:
- Conduct thorough and accurate assessments of patients' medical and functional needs
- Develop and implement individualized care plans that meet the unique needs of each patient
- Collaborate with healthcare professionals to ensure seamless care transitions and coordination
- Monitor and report on patient progress, identifying areas for improvement and implementing changes as needed
- Stay up-to-date on relevant regulations, laws, and industry standards, ensuring compliance and best practices
Requirements:
- Current and valid Texas RN nursing license or valid RN license from a Compact Party State
- Familiarity with reimbursement systems, including Medicare, Medicaid, and Case Management
- Preferred experience in Skilled Nursing Facilities as an MDS Nurse
- Knowledge of state and federal regulations, both clinical and financial, as it relates to the RAI process and reimbursement systems
