Ventricular Assist Device Coordinator RN

This position provides coordination and case management for ventricular assist devices (VAD) and cardiac transplant patients in collaboration with the program medical director and referring physicians to promote optimal patient outcomes.
Key Responsibilities- Coordinate and manage VAD and cardiac transplant patient care
- Collaborate with program medical director and referring physicians
- Develop and implement patient care plans
- Provide education and support to patients and families
- Participate in quality improvement initiatives
- Education to meet certification, license or registration requirement
- Oregon Registered Nurse License upon hire
- VAD coordinator experience
- 3 years - Clinical experience with VAD patient care
