This is a Physical Therapist position in the Home Care setting in the Newark, NJ area. You'll work with a geriatric population within their independent homes or within senior living/assisted living facilities.
Key Responsibilities- Provide direct patient care to geriatric patients in their homes or in senior living/assisted living facilities.
- Develop and implement individualized treatment plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure comprehensive care.
- Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with treatment plans.
