ROLE SUMMARY:Case Managers at Wellnest Emotional Health & Wellness provide a range of services and activities to support the program's goal of promoting children's emotional well-being. They conduct groups for children, high-risk teens, and adult parents of young children, focusing on building healthy relationships and promoting positive parenting. Case Managers promote a strengths-based approach to service, empowering families, supporting a community-based philosophy of service, and ensuring that all services and community events align with our mission, vision, and core values.
Essential Responsibilities:- Collaborate with therapists to conduct comprehensive needs assessments with children, youth, adults, and families to assist them in obtaining needed resources such as health insurance, housing, and low-cost childcare through referral and linkage.
- Provide individual and group rehabilitation services under Department of Mental Health contract or other agency funding.
- Meet agency productivity standards for client service hours.
- Complete and submit progress notes in a 24-hour timeframe using the agency's electronic health record system (EXYM) and collaborative documentation while serving clients.
- Provide guidance and assistance to parents/caretakers on managing their child's behaviors successfully.
- Provide guidance and assistance to parents/caretakers on managing familial stressors effectively and successfully.
- Assist families with identifying internal resources to assist with multiple stressors such as extended family members, grandparents.
- Develop and provide client rehabilitation services that assist in improving, maintaining, or restoring daily functioning at home, school, and community in accordance with treatment goals and in collaboration with the treatment team.
- Provide interagency and intra-agency consultation, communication, coordination, and referrals.
- Identify and communicate to therapists obstacles preventing families from connecting with the program and/or referral services.
- Attend community meetings as requested.
- Represent the agency at community resource fairs.
- Follow up with families that drop out of treatment.
- Provide services in the communities, in homes, schools, and other field-based settings.
- Participate in trainings and meetings as assigned.
- Other duties as assigned.
One to two years' experience providing case management services to low-income individuals in a community setting with an at-risk population. Bachelor's degree in social work, psychology, or other mental health-related discipline. Must possess and maintain a valid CA driver's license and insurable driving record that meets Wellnest's insurance carrier's criteria for liability coverage. Use of a personal vehicle is required to complete the essential duties of this position.
Conditions of Employment:Employee may be asked to pursue additional training when it is determined to be in the best interest of the Company.
Working Conditions:Employee regularly works out in the field and in an office environment. Employee may be required to work flexible, evening, and/or weekend hours for time-sensitive projects.

- Visual acuity, ability to adjust focus, and peripheral vision for work on a laptop or other computer equipment.
- Lift and/or move up to 25 pounds.
- Able to use telephone, office copier, calculator, fax machine, computer printer, and scanner, or other equipment as required.
- Annualized Salary Range: $47,098 to $52,919.
- This position is also eligible for an annual $3,000 bilingual Spanish differential.
