Job Description:

The ideal candidate will possess a graduate degree in Nursing or related field and 4-6 years of experience in care management. As a Healthcare Service Coordinator, you'll assess and coordinate medical and supporting services for complex populations, developing personalized care plans and addressing issues with members and their families.

Key Responsibilities:

  • Evaluate service needs and recommend plans for optimal outcomes
  • Develop and continuously assess ongoing long-term care plans
  • Coordinate between members, caregivers, and care providers to ensure adequate care
  • Monitor member status and revise care plans as necessary
  • Review member data to identify trends and improve operating performance

Required Skills and Qualifications:

  • Graduate degree in Nursing or related field
  • 4-6 years of experience in care management
  • RN licensure or NP certification
