Summary:

In this role, you will be responsible for delivering comprehensive case management services to a diverse range of clients, including vulnerable populations such as children, families, and individuals facing mental health challenges.

Key Responsibilities:

  • Provide clinical case management services through a structured short-term model, ensuring effective advocacy and crisis intervention.
  • Conduct regular meetings with clients and collaborate with community representatives to develop and monitor individualized treatment plans.
  • Engage in outreach activities and manage intake processes, creating tailored care plans that align with client needs.
  • Screen for income support and advocate for clients to access necessary benefits and entitlements.
  • Maintain accurate and timely documentation of case records and statistical reports.
  • Foster professional relationships with social service, health, and governmental agencies while adhering to confidentiality and ethical standards.
  • Participate in team meetings, training sessions, and program planning to enhance service delivery.
  • Meet project standards for work volume while integrating strengths-based approaches in clinical practice.
  • Ensure compliance with professional standards and participate in ongoing training to stay updated on best practices.

Qualifications:

  • Bachelor's degree in Social Work, Counseling, Clinical Psychology, or a related field, with three years of relevant experience, or a Master's degree.
  • Fluency in Spanish is highly preferred.

Certifications:

  • Possession of an LPC/LSW license may be required; certification as a Domestic Violence Professional is preferred.
  • A valid driver's license may be necessary.

Skills and Abilities:

  • Strong communication skills and the ability to work effectively with diverse populations.
  • Proficiency in Microsoft Office and familiarity with case management software.
  • Ability to manage confidential information with discretion and professionalism.
  • Capacity to work independently while maintaining motivation and focus.

Work Environment:

This position offers a hybrid work arrangement, requiring in-person attendance for meetings as needed. The role may involve travel to various program sites.

Physical Requirements:

  • Regularly required to communicate effectively and perform tasks that involve sitting, standing, and walking.
  • Must be able to lift and move items up to 25 pounds.
