Job Title: Lead YV Case Specialist

Job Summary:

The Lead YV Case Specialist will provide direct individual case management services to high-acute youth in the YV LifeSet program. This role requires a strong understanding of trauma-informed practice and the ability to work with youth, families, and community resources to achieve positive outcomes.

Key Responsibilities:

  • Provide case management services to assigned youth, including assessment, service planning, and implementation.
  • Develop and maintain relationships with community resources and service providers to ensure access to necessary services.
  • Collaborate with the YV LifeSet Case Specialist Supervisor to ensure compliance with program requirements and DCFS standards.
  • Provide leadership support in the absence of the Case Specialist Supervisor.
  • Attend training opportunities as required by UCAN, DCFS, and LifeSet.
  • Respond to critical incidents and unusual situations as required.

Requirements:

  • Bachelor's degree in Human Services, Psychology, or a related field.
  • Two to three years of case management experience, preferably in a youth-serving organization.
  • Valid Illinois driver's license and insurance, access to a car, and the ability to transport children.
  • Ability to work flexible hours, including evenings and weekends.

Working Environment:

This position requires local travel to provide services to youth at TLP placements and in their community. The Lead YV Case Specialist may be required to transport clients and/or their children and work on-call and weekends, evenings, and overtime.
