SOAR Case Manager Specialist

The successful candidate will be responsible for assisting individuals residing in Specialized Mental Health Rehabilitation Facilities and Nursing Facilities with applying for Social Security Benefits through the SOAR program.
Key Responsibilities- Conduct assessments to determine eligibility and gather supporting documentation
- Complete and submit necessary forms, and assist individuals in appealing social security administration decisions
- Engage participants in services using intensive outreach strategies
- Ensure timely access to services and ensure the safety of individuals
- Collaborate with other systems of care, including SMHRFs staff and treatment providers
- Document all services using program standards
- Bachelor's degree in psychology, social work, or a related field, or certification as a Certified Recovery Support Specialist (CRSS) or Certified Peer Recovery Specialist (CPRS)
- SOAR certification, or successful completion of SOAR training within 40 days of hire
- Experience working in mental health, substance use treatment, or I/DD services preferred
- Ability to work with diverse ethnic populations
- Personal vehicle, Illinois driver's license, and insurance, with a good driving record
- Ability to occasionally lift up to 35 pounds when applicable
