Job Summary

The Clinical Care Leader is responsible for utilizing the nursing process to provide individualized nursing care in the home setting. This role involves collaborating with inter-professional colleagues to plan, implement, and evaluate care. The Clinical Care Leader must have effective communication skills to collaborate, negotiate, and resolve conflicts with peers, inter-professional colleagues, residents, and families.

Key Responsibilities
  • Utilize the nursing process to provide individualized nursing care
  • Collaborate with inter-professional colleagues to plan, implement, and evaluate care
  • Have effective communication skills to collaborate, negotiate, and resolve conflicts
  • Have strong clinical reasoning and critical thinking skills to analyze data and respond to changing situations
Requirements
  • Bachelor's degree in nursing preferred
  • Minimum of two years' long-term care or geriatric nursing experience preferred
  • Currently holds an unencumbered RN license with the State Board of Nursing
  • Basic Life Support (BLS) required
