Job Summary

The University of Kansas Health System is seeking a skilled Clinical Nurse Specialist to join our team. As a key member of our Home Healthcare team, you will be responsible for coordinating patient care and promoting continuity through effective communication. You will work closely with patients, families, and healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Coordinate patient care and develop individualized care plans
  • Communicate effectively with patients, families, and healthcare professionals
  • Participate in interdisciplinary team meetings and contribute to the development of patient care plans
  • Monitor patient progress and adjust care plans as needed
  • Collaborate with other healthcare professionals to ensure seamless care transitions

Requirements
  • Associate Degree in Nursing
  • 1+ years of experience in Home Healthcare or a related field
  • Current RN licensure in the state of Kansas
  • CPR/AED/BLS certification

Preferred Qualifications
  • Bachelor's Degree in Nursing
  • 3+ years of experience in Home Healthcare or a related field
  • Medical-Surgical Nursing Certification
