Job Summary

The Clinical Care Leader is responsible for utilizing the nursing process to provide individualized care in the home setting. This role collaborates with inter-professional colleagues to plan, implement, and evaluate care, ensuring the highest quality of resident care.

Key Responsibilities

  • Utilize the nursing process to assess, diagnose, plan, implement, and evaluate resident care
  • Collaborate with inter-professional colleagues to plan and implement care
  • Document resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations
  • Utilize technology to enhance quality resident care and collaborate with inter-professional team members
  • Delegate to and supervise unlicensed personnel, assuming overall nursing accountability for a resident assignment

Requirements

  • Bachelor's degree in nursing preferred
  • Minimum of two years' long term care or geriatric nursing experience preferred
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring
