Responsibilities:

  • Analyzing assessment data to evaluate patient conditions and collaborating with providers to develop effective care plans
  • Providing education and support to patients, families, and staff members
  • Coordinating patient care with other healthcare entities
  • Administering medications safely and effectively
  • Serving as a subject matter expert and clinical mentor within the practice

Requirements:

  • CURRENT LPN LICENSE FROM THE STATE OF EMPLOYMENT OR LPN APPLICANT REQUIRED
  • BLS CERTIFICATION PREFERRED UPON HIRE AND REQUIRED WITHIN 30 DAYS OF INDEPENDENT PATIENT CARE
  • 1 YEAR OF CLINICAL PATIENT CARE EXPERIENCE PREFERRED
