EDUCATIONAL BACKGROUND, EXPERIENCE, AND TRAINING
- Must possess a Bachelor’s Degree from an accredited program recognized by the APTA (or equivalent international qualification), along with a valid and current license/certification to practice.
- Prior experience working with adult and elderly populations is essential.
- Experience in outpatient services and with pediatric/adolescent demographics is advantageous.
Participates in scheduled program inservice training sessions.
Engages in external professional development activities as arranged and shares insights with the team.
Assists the program director in identifying the educational requirements of the team and implements relevant inservice training sessions.
Adjusts personal schedules to accommodate program needs when necessary.
Maintains patient confidentiality at all times.
Communicates professionally with various stakeholders, as evidenced by client satisfaction.
Familiarizes oneself with and adheres to the relevant professional codes, company policies, program procedures, and hospital regulations.
Participates in community educational and promotional initiatives as requested.
Contributes positively to interdepartmental relationships and teamwork.
Ensures that all relevant licenses, certifications, and memberships are up to date.
Completes patient assessments and documents them in the medical records as per established policies and procedures.
Sets functional short and long-term goals that are objective and measurable.
a. Documents the timeline for achieving goals.
b. Selects interventions based on assessment findings that are appropriate for the patient's age, as evidenced by goal attainment.
c. Consults with physicians regarding referrals for additional services as necessary.
Ensures that therapeutic interventions yield positive outcomes, as determined by quality improvement assessments and patient feedback.
Demonstrates safety, competence, and proficiency in all departmental modalities and equipment usage.
Utilizes positive reinforcement to encourage patient engagement in their rehabilitation journey.
Provides structured and goal-oriented educational and training activities tailored to individual needs.
Collaborates with team members to coordinate therapeutic activities and interventions as required.
Evaluates patient responses to treatment plans, as evidenced by goal achievement and/or necessary revisions, and updates treatment plans accordingly.
Communicates current status, program planning, and completion details to patients and their families throughout the rehabilitation process.
Documents the patient care process, including progress and ongoing treatment needs.
- a. Records interventions and their effects daily, following policy and procedure.
- b. Completes interim progress notes in accordance with policy and procedure.
- c. Finalizes discharge summaries as per policy and procedure.
Acts as a resource for team members by providing inservice training on physical therapy knowledge and skills.
a. Attends inservice training sessions.
b. Leads inservice training sessions as requested or volunteered.
c. Orients new staff to the physical therapy functions within the rehabilitation department.
Participates in team meetings and contributes ideas and solutions in problem-solving discussions regarding the program.
Contributes to the development of improved efficiency and productivity in clinical and administrative functions of the program.
- a. Assists in planning and evaluation sessions with other rehabilitation staff, focusing on program development, organization, communication, and growth.
- b. Provides constructive suggestions to enhance service delivery and program operations.
- c. Collaborates in the implementation of system improvements.
- d. Offers assistance to other staff and demonstrates flexibility in fulfilling shared team responsibilities, including special projects.
