We are currently seeking a dedicated Physical Therapist to deliver essential services aimed at addressing impairments, functional limitations, disabilities, or alterations in physical function and health status.

Key Responsibilities:

  • Assist physicians or licensed practitioners in assessing patient functionality and determining equipment needs, while developing tailored care plans that include appropriate treatment goals and interventions.
  • Conduct initial and ongoing comprehensive assessments at designated intervals.
  • Collaborate in the formulation and evaluation of care plans alongside patients, their representatives, and caregivers.
  • Administer services as prescribed by the physician, adhering to the established care plan.
  • Implement therapeutic interventions in accordance with the physician-approved care plan and professional standards.
  • Regularly evaluate patient status and modify treatment plans as necessary, obtaining revised orders from the physician when indicated.
  • Consult with families and caregivers to facilitate patient progress towards agreed-upon objectives and discharge planning.
  • Communicate effectively with the interdisciplinary healthcare team to ensure coordinated patient care and discharge planning.
  • Document treatment and interventions accurately and timely, reflecting patient responses and progress in compliance with professional standards and policies.
  • Engage in Quality Assessment and Performance Improvement (QAPI) initiatives.
  • Participate in training and in-service sessions as required.
  • Contribute to the Office's Emergency Preparedness program as needed.
  • Supervise assigned professional and paraprofessional staff, including physical therapist assistants.
  • Access necessary personal health information (PHI) to fulfill job responsibilities.

Qualifications:

  • Current licensure as a physical therapist in the applicable state(s).
  • Preferred: At least 2 years of relevant experience as a physical therapist within the last 5 years.
  • Proof of current CPR certification as per organizational policy.

Skills and Competencies:

  • Proficient in using computers and workplace software.
  • Ability to work collaboratively under pressure, utilizing clinical judgment and creativity.
  • Effective communication skills in English across all workforce levels.
  • Adept at managing multiple tasks and priorities.
  • Ability to interpret technical instructions related to patient care and equipment usage.
  • Skilled in observing and assessing patients visually and aurally.
  • Capable of adapting to changes based on patient needs and scheduling.
  • Valid driver's license and auto liability insurance, if applicable.
  • Proof of Covid vaccination, medical exemption, or religious exemption.
