Cedarhurst of Frankfort, a dedicated provider of senior healthcare services, is in search of a compassionate and career-focused individual to join our team as a Resident Care Manager.

In the role of Resident Care Manager, you will oversee the coordination and success of our community care staff while enhancing the resident experience to ensure the highest standards of care.

Your contributions as a Resident Care Manager will include:

  • Taking charge, under the guidance of the Director of Nursing and Executive Director, for creating and implementing resident-centered programs tailored to the assessed needs, interests, and abilities of our residents.
  • Evaluating each resident's requirements upon admission and integrating activity goals into their personalized service plans.
  • Facilitating resident orientation and identifying potential adjustments or concerns within each resident's individualized service plan.
  • Collaborating effectively with staff, volunteers, residents, families, and the broader community to deliver a comprehensive psychosocial care model, aligned with our company's care philosophy.
  • Providing leadership and training to care staff and families regarding individualized service plans and best practices for supporting individuals with dementia.
  • Guiding staff and volunteers in specialized care training to address residents' needs and ensure the effective implementation of care programs.
  • Managing care staff schedules, delivering hands-on care as necessary, and ensuring adequate staffing levels.
  • Organizing and conducting orientation, mentorship, and ongoing training for care staff.
  • Guaranteeing exceptional resident care across all levels within the community.
  • Assisting in the preparation of monthly calendars and newsletters as needed.
  • Encouraging and participating in resident activities.
  • Engaging in the recruitment and onboarding process for care staff.
  • Providing feedback on care staff performance and collaborating with the Director of Nursing for performance evaluations.
  • Implementing training and orientation for new hires in the care staff team.
  • Conducting daily nursing responsibilities during scheduled shifts.
  • Understanding and executing all duties related to resident care roles effectively.
  • Promoting the physical, personal, and emotional well-being of each resident, striving to assist them in achieving their maximum level of independence and self-care.
  • Respecting residents' Advance Directives and adhering to their preferences.
  • Ensuring effective communication among community staff and residents' families regarding care, encompassing their physical, personal, and emotional well-being.
  • Demonstrating teamwork and following management direction while also taking initiative to address conflicts and resolve issues for residents, families, and staff as necessary.
  • Meeting the physical demands of the role, which may include standing, moving, communicating, and handling written information, as well as lifting and moving residents when required.
  • Performing additional duties as assigned.

To qualify for the Resident Care Manager position, you will need:

  • An Associate degree with 2+ years of relevant experience preferred, or 4+ years of applicable experience.
  • CNA or CMT certification is preferred.
  • CPR and BLS certification is preferred.
  • Proficiency in Internet browsers (e.g., Explorer, Chrome, Firefox), Microsoft Office applications including Outlook, Excel, Word, and Publisher, as well as electronic health records (EHR) systems and any applicable HR systems.
