Key Responsibilities- Therapeutic Interventions: Provide individual and group therapeutic interventions to participants, promoting positive outcomes and well-being.
- Treatment Planning: Develop, update, and monitor participant treatment plans, ensuring effective and personalized care.
- Assessments and Documentation: Complete program-specific assessments and document client treatment details in the Electronic Health Record, maintaining accurate and up-to-date records.
- Communication and Collaboration: Provide ongoing communication to the program team regarding participant changes, issues, or needs, ensuring seamless collaboration and coordination.
- Program Operations: Assist the program team as directed with the operation of the program, including discharge planning and referral.
- Compliance and Training: Maintain compliance by completing necessary and ongoing training, engaging in all staff meetings, and adhering to organizational policies and procedures.
- Education: Master's Degree in social services.
- Experience: 1 year of experience working with the target population, preferred.
- Certifications: Pass a Level II Background clearance.
