Job Purpose

As a Therapist at Aspire Health Partners, you will play a vital role in providing therapeutic interventions, creating treatment plans, and documenting client progress. Your expertise will help participants achieve their goals and improve their overall well-being.

Key Responsibilities
  • Provide individual and group therapeutic interventions to participants
  • Create, update, and monitor participant treatment plans
  • Complete program-specific assessments and document client treatment details
  • Communicate effectively with the program team regarding participant changes, issues, or needs
  • Assist with program operations and provide discharge planning and referrals
  • Maintain compliance with necessary training and staff meetings
Qualifications
  • Master's Degree in social services
  • 1 year experience working with the target population preferred
  • Pass a Level II Background clearance
